This case-based lecture outlines the clinical role of autologous serum tears in the management of ocular surface disease. It reviews what serum tears are, why they are used, which patients are ideal candidates, and when clinicians should consider prescribing them earlier in the treatment course. Case examples demonstrate practical decision-making in moderate-to-severe dry eye disease, aqueous-deficient dry eye, Sjögren disease, neurotrophic keratopathy, persistent epithelial defects, and post-surgical ocular surface compromise.
